COVID-19 Update 7/29/2020

We have been made aware of our 14th confirmed positive test result for COVID-19. This employee works out of the Seymour, WI location and has been absent from work since July 27, 2020. We’ve learned of 11 additional negative test results since the last letter was sent, bringing the total to 44 negative test results company-wide. Employees can be COVID tested at any of the following community testing sites in Green Bay, WI. All community testing sites are FREE of charge.

  • Prevea Health Center (2505 S Ashland Ave, Ashwaubenon). To test, you must register for a MyPrevea account which can be done at www.myprevea.com. Online scheduling available.
  • Aurora Baycare Medical Centers (Green Bay & Marinette sites). To test, you must register for a LiveWell or MyAdvocateAurora account which can be done at: myadvocateaurora.org/chart/signup
  • CVS Pharmacy (930 Main St, Green Bay). To test, visit www.cvs.com and click on the ‘Find Testing Sites’ button or call 920-436-9382.

COVID-19 Update 7/22/2020

We have been made aware of our 13th confirmed positive test result for COVID-19. This employee works out of the Seymour, WI location and has been absent from work since July 17, 2020. We’ve learned of 4 additional negative test results since the last letter was sent, bringing the total to 33 negative test results company-wide.

COVID-19 Update 6/24/2020

We have been made aware of our 9th,10th & 11th confirmed positive test results for COVID-19. All employees work out of the Seymour, WI location and have been absent from work since at least June 11th, 2020. We’ve also learned of 2 additional negative test results.

FACEMASK POLICY:
A. In an effort to reduce the spread of COVID-19, Performance Corp. has implemented a Facemask Policy requiring both company employees and visitors to wear face coverings. Face coverings should not be a substitute for social distancing, and individuals should continue to maintain at least 6ft distance between themselves and others whenever possible. Face coverings can consist of company provided face masks or personal cloth coverings, so long as they cover both the nose and mouth. Facemasks are not required while operating company owned vehicles (unless there is more than one person in the vehicle) and in certain approved situations where an employee’s work station is considered isolated from others for the majority of the work day. Employees with medical conditions that may cause them to be under duress should consult with their Supervisor or HR. If an
employee begins feeling overheated or is experiencing troubles breathing, they should remove their mask immediately and seek assistance from a Supervisor.

COVID-19 Update 6/15/2020

Dear Employees of Performance Corp.,

We have been made aware of our 7th and 8th confirmed positive test results for COVID-19. Both employees work out of the Seymour, WI location and have been absent from work since June 7th and June 8th, 2020 respectively. Since our last communication to employees on May 11, 2020 we have received an additional 17 negative test results bringing the total to 27.

UPDATED COMPANY ATTENDANCE POLICY REGARDING COVID-19:

Attendance/Illness Policy: Employees must report all illnesses or exposures to COVID-19 to their direct supervisor or HR immediately. Employees are required to stay home if they are ill or have been in contact with another person who has COVID-19 and should not attend work in any capacity if they are experiencing symptoms of the coronavirus (see specific coronavirus symptoms by visiting cdc.gov).  Employees who call-in sick will be asked to provide information in relation to their specific symptoms and direct contacts in an effort to identify risk to other employees. Employees who exhibit symptoms of coronavirus will be required to self-isolate for a period of up to 10 days and until asymptomatic for a period of up to 72 hours. Employees identified as direct contacts may also be required to self-isolate for up to 14 days. Employees will be encouraged to conduct a symptom checker online, seek medical attention and/or get tested for the virus if they are exhibiting symptoms. Employees who present signs of illness while at work will be separated from other employees and sent home immediately. Employees should avoid using Emergency Care resources unless necessary. In order to receive Sick Pay for COVID related absences, Performance Corp. will request documentation. Employees who live with or have a chronic health condition should take additional steps to reduce public presence and should communicate with HR if they feel the need to be quarantined for their own safety.

COVID-19 Update 5/11/2020

On May 8th and May 10th, 2020, we became aware of our 5th and 6th confirmed positive test results for COVID-19. Both employees work out of the Seymour, WI location on A Shift and have been absent from work since May 4th and April 30th, 2020 respectively. Over the weekend we also learned of 5 additional negative test results, bringing our total negative results to 10 company-wide.

Employees of Performance Corp. can go to the following locations for FREE testing. Please keep in mind that tests done at Emergency Rooms or other locations may come at a cost. All locations have interpreters on-site.

On May 5, 2020 we became aware of a fourth employee whom tested positive for COVID-19. This employee works out of the Seymour, WI location on A Shift and has been absent from work since April 27th, 2020. We are continually cleaning and disinfecting all areas of the facility and expect employees to follow all social distancing procedures and personal hygiene measures to help prevent the spread of the virus.

COVID-19 Update 4/30/2020

Dear Employees of Performance Corp.,

On April 29, 2020 we became aware of a third employee whom tested positive for COVID-19. This employee works out of the Seymour, WI location on A Shift and has been absent from work since April 21st, 2020. We continue to believe these cases are not from internal contact, however we are continually cleaning and disinfecting all areas of the facility and expect employees to follow all social distancing procedures and personal hygiene measures to help prevent the spread of the virus.

COVID-19 Update 4/23/2020

I am writing to inform you of our 2nd confirmed positive COVID‐19 test out of our Seymour, WI location. We received notification of the positive case Wednesday, 4/22. This employee has been in self‐quarantine since Wednesday, April 15th. We have strong belief that this case, similar to the first case, originated outside of Performance Corp. We continue to believe these cases are NOT from community spread within our facilities. We have also received confirmation of one negative test. Any employee identified as a close contact to the individual’s who have tested positive have been communicated to, to inform them of their exposure and need to self‐quarantine.

In total, we have:

  • 2 employees with positive coronavirus tests; outside exposure
  • 1 employee who has tested negative
  • 1 employee with a pending test result in Seymour, and 1 employee with a pending test result in Carney.

All other employees who are away from work have been self‐quarantined as a precautionary measure.

In recognition of the efforts of those employees who continue to work on the frontlines during the coronavirus pandemic, we have decided to install a temporary Frontline Employee Incentive program which will provide a $3.00 per hour incentive to each individual’s hourly rate, effective Sunday, April 26, 2020. For more details on the incentive pay, please refer to the Frontline Employee Incentive program posted on company bulletin boards.

Additionally, on the back of this letter, you will find a listing of the safety measures and efforts we have put in place as a company. We are adding new items to the list regularly and encourage employees to share their ideas with their Supervisors or by sharing them to the HR Department directly through the Employee Portal.

COVID-19 Update 4/20/2020

I am writing to inform you of our first and only confirmed positive COVID-19 test out of our Seymour, WI location. We received notification of the positive case Sunday night (4/19). This employee has been in self-quarantine since Monday, April 13th as a result of being in contact with an outside person who tested positive. Due to the outside contact and the fact that we do not currently have any other employee’s being tested at this location, we believe this is not a community spread situation.

We have confirmed and communicated with all individuals identified as close contacts, and have asked them to self-quarantine for the required timeframe. We understand that the employee whom tested
positive was actively following safe-distancing practices and was wearing a face mask while at work for at least 2 weeks.

Performance Corp. has been actively following the CDC and WHO recommendations in response to the coronavirus pandemic, and will continue to implement any measures possible and reasonable to ensure
your safety during this unprecedented time. All areas within the facilities are continually being disinfected, and work stations are provided with cleaning supplies to allow for additional precautionary cleaning. All employees are required to abide by social distancing expectations (6ft), are mandated to wear either a company provided or personal face mask, are encouraged to increase hand washing, and limit touching their face. Any employee who is experiencing symptoms of respiratory illness or who has been exposed to someone positive with COVID-19 should not report to work and should call Tori Maass.

THANK YOU to those who continue to work on the front-lines every day. Our country depends on products being shipped to grocery stores, hospitals and other essential businesses – your work to ensure that happens is not going unnoticed.

Face Masks Required for All Performance Employees

In light of the continual Coronavirus pandemic and cases within our area, Performance Corp. will be mandating that all on-site employees wear face masks effective Monday, April 20. This is in an effort to prevent the spread of the virus within our organization and to keep true to our goal of keeping all employees safe from contracting COVID-19. It is our strong recommendation that all employees find and use their own fabric face mask (see attached template) for any travel outside of the home, including trips to the store, to work or other locations that are deemed essential.  We have already placed an order for disposable face masks and anticipate receiving the shipment of these masks on Monday 4/20. That being said, wearing any type of face mask is NOT a substitution for social distancing and employees are still required to provide 6-feet distance between themselves and others.  

At this time, we still do not have any confirmed positive cases of coronavirus within our workforce.

We appreciate those who are continually working on the front lines and will continue to communicate with everyone regarding company efforts to combat this virus.
